(a) Filing. A notice of removal must be accompanied by:
(1) all documents required by 28 U.S.C. § 1446(a);
(2) the filing fee payable to “Clerk, U.S. District Court”; and
(3) a Civil Cover Sheet [form JS 44].
(b) State Court Record. The clerk may request the complete file directly from the state court
and instruct it to bill the removing attorney for any associated costs.
