(a) Summons. Subject to Fed. R. Civ. P. 4(d), the plaintiff may request a formal summons from
the court [AO 440 or Fed. R. Civ. P. Form 3].
(b) Third-Party Summons. A defending party may request a third-party summons from the
court [AO 441 or Fed. R. Civ. P. Form 4].
