Revised Civil Cover Sheet
The United States District Court for the Southern District of New York has revised the court’s Civil
Cover Sheet (form JS 44C/SDNY). Parties are advised to discard copies of older forms.
The form has been modified to reorganize the Nature of Suit Code section to help filers locate the
proper Code. A separate guide, Civil Nature of Suit Code Descriptions, has also been added to assist
in selecting the appropriate code.
Filers are reminded the Nature of Suit Code selected on the Civil Cover Sheet must match the code
entered on the ECF screen when filing a new case.
