A party demanding a jury trial in a pleading as permitted by Fed. R. Civ. P. 38(b)
must include the demand in the title by way of a notation placed on the front page
of the pleading, immediately following the title of the pleading, stating “Demand for
Jury Trial.” Failure to do so will not result in a waiver under Rule 38(d) if a jury
demand is otherwise properly filed and served under Rule 38(b).
